For your convenience we have made our patient forms available to you. We recommend printing and completing these forms in the comfort of your home. Please remember to bring your completed forms with you for your first appointment.

Financial Arrangements

We ask all new patients to pay in full even if you have dental insurance. We will be happy to assist you in filing a claim for your reimbursement. We accept MasterCard, Visa, Discover, American Express, cash and personal checks. If you prefer to have a payment plan, we can also assist you in making extended financial arrangements with our finance company, Care Credit.
